Evidence-Based Medicine (EBM)

Student/Resident/Reviewer:  Jon Smith
Status:   New

This practicum is designed to give you "hands on" experience in the process of EBM with mentor supervision. Specifically, you will carry out the first four processes of EBM:
I. Ask a question: Based on a patient encounter, and with your mentor's guidance, choose a focused and answerable clinical question.
II. Acquire the evidence: With the help of a reference librarian, perform a literature search designed to answer your question with the "best" available evidence.
III. Appraise the evidence: Use the JAMA series "User's Guide to the Medical Literature" to critically evaluate the article(s) that resulted from your search, with specific focus on the following:
a. Statement of purpose
b. Sample (with particular attention to inclusion and exclusion criteria)
c. Methods (with particular attention to the appropriateness of design, data collection, and statistical analyses)
d. Description of results
e. Appropriateness of conclusions based on data
IV. Apply the evidence to your individual patient.
Once this process is complete, the results should be presented to your peers at a noon conference. Please make an effort to clarify EBM terms (e.g. RCT, cohort study, case-control study, relative risk, odds ratio, number needed to treat, maximum likelihood ratio, etc.)

Criterion
1. Asking a Clinical Question
Questions Vague and/or unanswerable Questions clear and answerable
2. Acquiring the evidence
Search strategies unable to yield answers Search Strategies yield useful studies to answer the clinical question
3. Appraising the evidence
Misinterprets the major findings Interprets the major findings properly but misses the subtleties Correctly interprets both the major findings and subtleties in the evidence
4. Applying the evidence to the care of one's patient
Incorrectly applies the evidence to the patient Correctly applies the evidence to the patient

Criteria for Overall Assessment:

Below Standards: Two or more criteria with a below standards rating results in an overall evaluation of below standards and requires repeat of the exercise

Meets Standards: Learner must receive a
meets standards for at least 3 of the 4 criteria, including appraising the information

Exceeds Standards: Learner must receive a
meets standards in asking a clinical question, acquiring the information, and applying the information, and an exceeds standards in appraising the evidence.
